Jeremy Gardner accepts Highmark Blue Cross Blue Shield. Insurance rates apply for eligible members.

Insurance credentialing in progress: Jeremy is currently in the process of credentialing with Medicaid and additional commercial insurance plans. Once credentialing is complete, those plans will be accepted for medication management. Contact us for the most current information on accepted coverage.

Your Right to a Good Faith Estimate

Under the No Surprises Act (effective January 1, 2022), you have the right to receive a Good Faith Estimate of expected costs before beginning services. As a healthcare provider, Open Minds Therapy is required by law to provide a written Good Faith Estimate to any client who is uninsured or who chooses not to use their insurance.

Your estimate will include the expected cost of services based on your anticipated treatment needs, including session fees and projected frequency. You have the right to receive this estimate at least one business day before your first scheduled appointment.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you may be eligible to dispute the charges. For questions about your right to a Good Faith Estimate or to request one, contact us at (302) 751-2003 or [email protected].
